How long does the process take?
It depends on your situation and how quickly records can be gathered. After you submit, we’ll help you prioritize the most important next step.
Can family members fill this out for a former DOE worker?

Is there a cost to submit the Pathway to Care form?
There is no cost to submit the form and begin the intake process. If any paid services apply later, we’ll explain them before you decide.

We use your information to review your situation and follow up with next steps. Our systems are HIPAA-compliant, but please avoid sharing highly sensitive details you don’t want transmitted online. We can collect additional details on the phone if needed.
What DOE sites do you work with?
We support former workers from EEOICPA-approved DOE sites and contractor environments. If you don’t see your site listed elsewhere on the website, submit anyway and include the site name you remember.
