DOL White Card

Let's Get Started

  1. We first need to determine if you or a loved one qualify for EEOICPA or RECA benefits to obtain a Department of Labor (DOL) White Card. Complete the form below and one of our Community Outreach Specialists will follow up with you.

  2. If you or a loved one qualify for benefits, the steps below the form provide a general outline of what happens next and how we can help.

  3. If you or a loved one already have a DOL White Card and currently receive home healthcare but wish to transfer care to us, please indicate on the form.

If you or a loved one qualify for benefits, these steps provide a general outline of what happens next and how we can help.

Initial Meeting

We meet with you to understand your needs and discuss your pathway to receiving home healthcare with Trusted Ally.


 

White Card: Yes

If the DOL White Card has been issued, our Clinical Team meets with you to:

  • Perform a medical evaluation
  • Create a Plan of Care based on the evaluation
  • Review Durable Medical equipment
  • Coordinate a visit with your Treating Physician
  • Submit information to the Department of Labor

 

Approval

Once the information has been submitted to the DOL, it typically takes 60-100 days for approval, but if care is being transferred to Trusted Ally, the approval is usually immediate.


 

Home Care Begins

After approval, Trusted Ally Home Care begins!

  • Admission visit
  • Staff meet and greet

 

Ongoing Evaluation

Going forward, we continue to evaluate your needs by:

  • Monitoring home healthcare services
  • Supporting you every step of the way with a comprehensive and holistic approach
  • Ensuring all Durable Medical needs are met
  • Working with your Authorized Representative to maximize financial benefits

 

White Card: No

If the DOL White Card has not yet been issued or denied, we will connect you with an Authorized Representative (AR) and they will fight for your benefits:

  • They will gather your medical records and do a case review
  • They help you file your claim
  • If needed, they will represent you at a case hearing
  • The process typically takes about 6 to 9 months
